Use the HIPAA-compliant referral form at WA Community Living Connections. It will go directly to the Aging & Disability Resource Center in the county where your patient lives.

Primary Care Liaison

The Primary Care Liaison is a single point of contact for busy providers. The Primary Care Liaison can help answer questions about long-term care services, staff challenging cases,  provide follow-up on referrals, and help you connect to evidence-based programs and services for your patients.

Our partners at the Northwest Geriatrics Workforce Enhancement Center offer an online lecture series led by nationally and regionally recognized geriatrics experts.
