What is the best way to refer a patient?+
Use the HIPAA-compliant referral form at WA Community Living Connections. It will go directly to the Aging & Disability Resource Center in the county where your patient lives.
How do I obtain follow up on my referral?+
Contact our Primary Care Liaison to learn the outcome of a referral you sent to our agency.
How long until someone contacts my patient?+
Your patient will be contacted by our Aging & Disability Resource Center Team within two business days.
Can I get brochures or flyers to give to my patients?+
Yes, we would be happy to send some of our Agency rack cards to your clinic. Please connect with our Primary Care Liaison to submit your request.
Does my patient already have a case manager?+
If your patient has a case manager, our Primary Care Liaison can help put you in touch with the case manager.
